A 40 year old female patient with history of bilateral hip osteoarthritis and severe lumbar and cervical ankylosing spondylitis posted for right THR . It was difficult spinal as patient was not able to give proper position for anaesthesia. Intubation was difficult because of severe cervical spondylitis and no movement at neck. Both spinal and general anaesthesia were practically not possible. So we planned to give spinal anaesthesia with a different approach that is Taylor’s approach using C- arm guidance. Adequate effect of spinal anaesthesia achieved and surgery went uneventful.
